Incident Summary: Kone Elevators Pty Ltd (“Kone”) was engaged to perform maintenance and repairs on a lift at the Sofitel Hotel. The lift was the goods and services lift used by staff of the Hotel. On 5 June 2006, the lift failed whilst it was being used by a person working at the Sofitel but was stopped by the overspeed governor and stand alone safety system. The person was thrown to the floor and asserts he suffered injuries to his back and shock. The counterweight for the lift separated from the hoist ropes and fell a distance of approximately 137 metres, crashing through the lift pit into the void below and destroying a number of components in its path. Kone were aware of the hazards associated with this type of traction lift after preparing a letter to lift-owners in 2001 describing the risk of failure of the counterweight assembly and preparing a risk management form. Employees of Kone directed to carry out maintenance on the said lift prior to the incident were not provided with a copy of the risk management form, or standard drawings for the sheave assembly or the written procedures in relation to pre-load testing of tapered roller bearings

A UNION has called for tighter regulation of lift maintenance after two men came forward yesterday with stories of terrifying experiences in major city buildings.
Chef Glenn Rochester was in a goods lift at the Sofitel Hotel on Collins Street last Monday when a counterweight, estimated to weigh two tonnes, snapped off and plummeted to the bottom of the lift shaft, causing the lift to drop before safety brakes kicked in. Mr Rochester said he fell 15 floors and suffered back and arm injuries.
In February, David Anesbury was in a lift in the Sofitel's twin building, occupied by ANZ Bank, that fell up to 15 floors, leaving occupants distressed and visibly shaken.
The incident at the Sofitel is being investigated by WorkSafe, building owner AMP Capital Investors and Kone, the maker of the lift. A spokesman for WorkSafe said he was unaware how Mr Rochester had calculated the number of floors he fell, given that he began his journey on the below ground P2 level and finished it on the ground floor.
The spokesman said the investigation was under way and would focus on how the counterweight snapped off. He said the safety features on the lift had worked as they should.

Sofitel refused to comment for this story, while a spokesman for AMP Capital Investors said it was too early to anticipate the outcome of the investigation.
Mr Anesbury, a visitor to the building, said that after his experience in February, passengers reported the plunge to the reception desk at 55 Collins Street, but a spokesman for ANZ Bank said it had no reports of the incident, conceding that communications between the reception desk and the bank may have broken down.
"We'll certainly go back and review our communications processes between the front desk and our maintenance company if there is a suggestion that that could be at fault," he said.
Jane Farrell, the assistant secretary of the Liquor, Hospitality and Miscellaneous Union, said there should be closer regulation of lift maintenance.
"There needs to be somebody overseeing the people who claim that they're checking buildings, making them safe, the upkeeping and maintenance, putting new parts in," she said.
"We want to make sure there's a government authority saying 'tick, yes they are doing that.' That is a lot more complicated than me just calling for it, though."
While WorkSafe sets regulations and conducts periodic random checks, responsibility for health and safety rests with the companies. A WorkSafe spokesman said it would be too big a job to monitor all 3500 registered lifts in the state.
"The reality is, nobody will ever get to all of those (lifts). What sort of army would you require to check them? That's why the owners and occupiers have a very clear legal responsibility," he said.
AMP Capital Investors hires an outside business to monitor Kone's maintenance of the lift.

ELA, the voice of the lift, escalator and moving walk industry in Europe
ELA, the European Lift Association, represents the lifts, escalators and moving walks associations active in the European Union (EU) or the European Free Trade Area (EFTA), whether they are national associations or sector specific associations. ELA also represents their components manufacturers. Thanks to this very wide representation,
ELA has become the main communication vector of the lift, escalator and moving walkways industry to the European Commission, the European Parliament and a range of other institutions and organizations. Its aim is also to help the national associations in their dialogue with their respective governments.

ELA is the result of the transformation of the European Forum of the National Lift Associations (EFLA) into the wider ranging European Lift Association, in the course of 2002. The by-laws changes have been published in the Belgian official journal (Belgisch Staatsblad - Moniteur Belge) on September 25th, 2002.

ELA welcomes EEA, the European Elevator Association, as one of its members, next to the other associations representing the world of lifts in Europe.

固有升降機安全規範(Safety Norms for Existing Lifts, SNEL)概覽

什麼是SNEL?
在2003以前,當時稱為EN-81的升降機歐洲標準,與其他國際標準大致相等。該標準的一個原則,是升降機的安裝必須符合當時有效的法規;然而,假若日後頒布了新的安全法規,則原已安裝的升降機毋須遵守。這個原則導致了安全漏洞,並且隨著更多新法規的頒布,差距愈來愈大。

目前,歐洲共有逾300萬台升降機正在運作,當中逾半數的機齡為20 年或以上。有鑑於此,來自升降機行業、政府機關、獨立檢驗機構、消費者組織及保險公司等範疇的安全專家,多年來緊密合作,終於促使歐盟就固有升降機的安全制訂主要標準,名為「固有升降機安全規範」(SNEL)或EN81之第80部分(Part 80)。這個標準把不同風險及危險情況加以分類,並提供詳細的修正行動,藉此消除風險及改善安全。該標準於2003年被納入當時的EN81標準。

落實SNEL標準有何益處?
由於以前的安裝法規,沒有規定固有升降機須就新安全法規進行升級工程,結果使安全漏洞逐年遞增;而SNEL標準的落實,正可把有關漏洞加以堵塞。該項標準考慮到成熟經濟體/市場有許多使用經年的升降機,而它們的安全標準難免不合時宜。SNEL要求固有升降機必須遵守最新的安全法規,成功消除了種種安全風險,減低發生意外的機會,因而有助改善公眾及工地的安全。

哪些歐洲國家採納了SNEL標準?
法國、比利時、奧地利、西班牙、意大利、希臘、瑞典、德國、盧森堡及馬耳他等國,均落實規例,要求大廈業主在指定時間內逐步遵守SNEL標準。

香港政府有否採納SNEL標準?
自2007年起,香港把SNEL制訂成自願性實務守則;政府鼓勵大廈業主採納SNEL標準,但沒有立法強制執行。
